Your Mac is on the monitor Use this screen as a trackpad: slide to move, tap to click, two fingers to scroll, touch and hold then slide to drag. Your keyboard and mouse control the Mac: this screen stands for the Mac's whole screen. Three-finger tap (or Ctrl+Option+M) for the menu.
The Mac has no screen to show right now. If the lid is closed, press Ctrl + Option + M (or tap with three fingers) and turn on 2nd screen or Lid closed.

Make sure MacRemote is running on your Mac and both are on the same Wi-Fi. Once connected, press Ctrl + Option + M or tap with three fingers for the menu.

Open this menu any time with Ctrl + Option + M on the keyboard, or tap the screen with three fingers.
Sound
Plays the Mac's sound on this iPad, or on the earphones connected to it.
iPhone as trackpad
Scan with the iPhone camera, or open in Safari on the iPhone. Then tap Share, Add to Home Screen.

Second screen
Tap 2nd screen to use the iPad as an extra screen for your Mac. Move the pointer or drag windows off that side of the Mac's screen to bring them onto the iPad.
Mac screen
Lid closed keeps the Mac awake with the lid shut. The first time, the Mac asks for your password (only once, ever). After that it stays on whenever MacRemote runs, and when you close the lid the screen switches over by itself.

Mouse buttons
Gesture button: hold it and move the mouse up for Mission Control, down for App Exposé, or left and right to switch desktop. A quick click does its normal job.
